Dedication & Acknowledgment

“All praise is due to Allah, by whose grace good deeds are accomplished…”

We dedicate this work to our parents, families, and respected professors at the College of Dentistry, University of Science and Technology, who illuminated our path with knowledge and guided our clinical foundation.

01. Academic & Clinical Journey

During the past three years of studying dentistry, we experienced an intensive academic and clinical journey. Moving from theoretical learning to hands-on practice, we acquired expertise across primary dental modalities: Preventive Dentistry, Periodontics, Extractions, Operative Restorations, Endodontics, Fixed Prosthodontics, and Removable Dentures.

1. Oral Hygiene & Periodontal Care

Oral hygiene is the cornerstone of overall health. Neglecting oral cleanliness leads to plaque accumulation, hardening into tartar (calculus), causing gingivitis and periodontitis.

Clinical Cleaning Workflow

1. Examination
Diagnosis & visual assessment before treatment.
2. Motivation & Edu
Inspiring habits and explaining disease etiology.
3. Instruction
Teaching Bass technique & interdental flossing.
4. Scaling & Polish
Ultrasonic deposit removal & prophy polishing.

Clinical Case Record — Periodontics

Patient Name: Gamela Abdullah Age / Sex: 39 Yrs / Female
Chief Complaint: Desires improvement in aesthetic appearance of teeth.
Diagnosis: Plaque-induced gingivitis with localized periodontitis.

2. Non-Surgical Oral Surgery & Extractions

Non-surgical tooth removal relies on progressive expansion of the bony socket. Precise anatomical evaluation between Maxilla (thinner cortex, vascular) and Mandible (denser bone, thicker cortices) dictates clinical strategy.

Jaw Structure Bone Density Anesthesia & Healing
Maxilla (Upper Jaw) Thinner, softer cortical plates (buccal < palatal). Excellent infiltration penetration; rapid vascular healing.
Mandible (Lower Jaw) Thicker cortices, dense trabecular structure. Requires block anesthesia (IDNB); slower bone healing.

Key Extraction Indications & Contraindications

  • Primary Indications: Severe unrestorable caries, pulpal/periapical pathology, advanced mobility (periodontitis), impacted or supernumerary teeth, pre-radiotherapy clearing.
  • Medical Precautions: Patients on Antiresorptive Agents (Bisphosphonates) carry risk of MRONJ; patients post-head/neck radiation risk Osteoradionecrosis (ORN).

Clinical Case Record — Oral Surgery

Patient Name: Ahmed Mohamed Age / Sex: 80 Yrs / Male
Medical History: Hypertension (Controlled via Concor 10mg daily).
Planned Procedure: Nonsurgical extraction of Maxillary left second molar under LA with socket squeezing & debridement.

3. Operative & Restorative Dentistry

Operative dentistry aims to restore function, integrity, and aesthetics using minimally invasive techniques. Modern restoration emphasizes adhesive bonding with Composite Resin following precise cavity preparation.

Restorative Protocol

  1. Diagnosis & Anesthesia: Vitality testing, clinical exam, local administration.
  2. Isolation: Field isolation utilizing Rubber Dam kit or cotton rolls.
  3. Caries Excavation & Preparation: High/Low-speed removal, ensuring sound dentinal margins.
  4. Etching & Bonding: 37% Phosphoric acid application followed by adhesive curing.
  5. Incremental Placement & Light Curing: Layered resin adaptation to minimize polymerization shrinkage.
  6. Finishing & Occlusal Polishing: Adjusting contact points and achieving smooth margins.

Clinical Case Record — Operative

Patient Name: Hanaa Age / Sex: 35 Yrs / Female
Chief Complaint: Cold water sensitivity. Systemically free.
Diagnosis / Ttt: Reversible Pulpitis — Direct Light-Cured Composite Restoration.

4. Endodontics & Intracanal Fiber Post Reconstruction

Endodontic therapy isolates, cleans, shapes, and hermetically obturates the canal system. In cases of extensive coronal loss, Glass Fiber Posts are selected due to their modulus of elasticity matching dentin, reducing catastrophic root fracture risks.

Diagnostic Differential: Pulpitis vs. Periodontitis

Feature Pulpitis Periodontitis
Pain Profile Sharp, spontaneous, thermal-triggered. Dull, localized, mastication-induced.
Percussion / Mobility Negative / Normal mobility. Positive percussion / Increased mobility.

Fiber Post Sizing Applied (Clinical Case):

Lower Right (#2, #3, #4, #5): Size 20 | Lower Left (#2): Size 20 | Lower Left (#3): Size 30 | Lower Left (#4): Size 25.

Clinical Case Record — Endodontics & Post Core

Patient Name: Fatimah Ahmed Age / Sex: 35 Yrs / Female
Diagnosis: Grossly destructed lower anterior & premolar teeth.
Treatment Plan: RCT (Step-Back Technique) + Intracanal Fiber Post + Core Build-Up + Crown.

5. Fixed & Removable Prosthodontics

Fixed Prosthodontics (Crowns)

Restoring form, function, and aesthetics of compromised teeth permanently.

  • Tooth reduction respecting axial taper & finish lines.
  • Precise elastomer impression & bite registration.
  • Temporary protection & luting cementation.

Removable Dentures

Rehabilitating completely edentulous arches for function & facial support.

  • Primary cast & custom acrylic tray fabrication.
  • Green-stick border molding & ZOE final wash.
  • Jaw relation recording & wax try-in evaluation.

Clinical Case Record — Complete Denture

Patient Name: Nabil Tawfiq Age / Sex: 50 Yrs / Male (Teacher)
Chief Complaint: Difficulty masticating food and aesthetic concerns during teaching.
Diagnosis / Arch: Completely edentulous maxillary (V-shaped) and mandibular (U-shaped) ridges.

Conclusion & Clinical Reflection

Integrating rigorous theoretical knowledge with direct hands-on clinical patient management represents the foundation of professional dentistry. Across these clinical cases, mastering precision, strict sterilization protocol, compassionate patient communication, and meticulous clinical execution has built our confidence to deliver high-standard oral healthcare to our community.
